L’Immacolata in Mafia The Old Country is the first chapter of the brand-new Man of Honor DLC. It tasks you with tracking down an NPC named Andrea, who has been stealing wine from the Don. You must locate him and teach him a lesson in the first mission of the expansion.

How to complete the L’Immacolata quest in Mafia The Old Country Man of Honor DLC
Reach the cellar
The L’Immacolata mission in Mafia The Old Country Man of Honor DLC begins with a cutscene of Enzo informing Valerio about Andrea’s antics. Following that, disembark from the car and select your weapons from the available options on the crate.
Once you’re ready, follow the quest marker toward the building on the opposite side of the road. Upon arriving, sneakily proceed inside toward the cellar where Andrea is without being spotted by other NPCs. You can use a knife to kill them without making a noise.
Go down to the cellar and find Andrea
Upon arriving, open the door to the cellar and then proceed downstairs. Now sneak around the cellar while killing off Andrea’s men one by one without being noticed. After a certain point, it will be revealed that Andrea is busy with some paperwork, and the buyer is asked to wait for him.
Finish off the buyer and other NPCs and proceed toward the wooden door highlighted by the quest marker. Open it to find Andrea on the other side and trigger another cutscene where the thief pleads for mercy but is eventually shot by Enzo.
Leave the cellar
The next objective of the L’Immacolata quest in Mafia The Old Country is leaving the cellar. Simply head back the way you came from to find Cesare by the stairs. Now follow him outside to Andrea’s car and start it.
Meet with Luca in San Celeste
The next part of this Man of Honor DLC mission is driving Andrea’s car to San Celeste to meet up with Luca. After reaching the location, get out of the car and take a walk with Luca and fill him in on the situation.
Complete tasks
Once your interaction with Luca is finished, head over to the market to collect pizzo from the hat maker. You can simply follow the quest marker to find the quest location.
After the interaction, visit Signor LaPaglia’s home to collect an envelope with money. After that, follow the quest marker toward a car.
Your next destination in Chapter 1 of the new Mafia The Old Country DLC is Pasquale’s house. Simply drive the car for around 400 meters to reach the spot. Now speak with Pasquale, and then drive the car back to the vineyard next, which is around 900 meters away, to reach Cesare.
After interacting with him, go to Tino to give him the pizzo. He will then ask you to deliver it to Father Ciccone and head to the Don’s office.
Go to Don Torrisi’s office
After a brief chat with Isabella, make your way to Don Torrisi’s office to meet him. As you enter, another cutscene will trigger where Ennio Salieri will make his appearance. He will inform the Don of two men who have stolen from him.
Following this, meet up with Cesare and Salieri outside. Then go to the garage and bring out a car. Once Cesare and Salieri get inside, drive off to Dante’s farm, which is around 2.3 kilometers away.
Help Salieri find the horses
Upon arriving at Dante’s farm in the L’Immacolata mission of Mafia The Old Country Man of Honor DLC, disembark from the car and follow Salieri to the farm. You must now sneak inside through a hole in the wall covered by a beam.
Afterward, kill Dante’s men on the grounds without making a noise. Then head toward the marked building to look for the horses, only to find them not there.
At that point, Salieri will ask you to spread out and look for the horses. Now follow the quest marker toward the main stable. Do note that you will have to deal with several enemies along the way.
Once you have looked inside the stable, proceed toward the barn next while killing the guards in the way. Do note that the door to the barn will be locked, and you will have to jump inside it from the hole in the wall on the first floor.
Your next task is to investigate the barn. After gathering info, unbar the door to the barn to begin a cutscene.
Kill Dante’s men
As soon as the cutscene ends, Dante’s men will arrive at the location and begin shooting. Take cover and shoot back at them to defeat them. After finishing them off, regroup with Salieri in the barn.
Following a brief chat, follow Cesare and help him dump the dead bodies inside a hut.
Go to the warehouse in Porto Almaro
After taking care of all the dead bodies in the L’Immacolata quest of Mafia The Old Country Man of Honor DLC, walk to the truck with Cesare. Start it and then drive toward a warehouse in Porto Almaro by following the quest marker.
Now, get off the truck and talk to Salieri’s man outside the warehouse. Then follow Cesare to a bar nearby to trigger the final cutscene of the mission.
